Students must provide the correct Ƶ email for the instructor in the instructor email column. If an incorrect instructor email is submitted, the form will not be processed. Forms that are deemed to be incomplete/incorrect may not be considered for review.

This request is only for the CURRENT term courses that have a final grade of NF or verified as never attended. THIS FORM CAN ONLY BE USED DUE TO NEVER ATTENDING.
If this exception is approved, you will be officially withdrawn (without a W), as a result of never attending the course, resulting in a 100% tuition credit for the particular course(s).
Please note a tuition credit may not result in a credit on your student account.
